What is Pulmonary Vein Isolation in AF Ablation?

Pulmonary vein isolation (PVI) is the procedure of electrically isolating pulmonary vein foci, which are triggers of atrial fibrillation, from the left atrium. It is performed with RF ablation or cryoablation (balloon).

In paroxysmal AF, single-procedure ablation success is 70-80%. Persistent AF may require additional substrate modification. Superiority over antiarrhythmic drug therapy has been proven.

Symptoms

Symptomatic atrial fibrillation episodes
AF refractory to antiarrhythmic drug therapy
AF with heart failure (may improve EF)
Tachycardia-induced cardiomyopathy

Risk Factors

Left atrial dilatation (>50 mm — lower success rate)
Long-standing persistent AF
Obesity (increased recurrence risk)
Untreated obstructive sleep apnea (increases recurrence)
Advanced age

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Symptomatic paroxysmal AF (unresponsive to one antiarrhythmic)
  • Symptomatic persistent AF
  • AF with heart failure (low EF)
  • When ablation is preferred as first-line treatment

Procedure duration 2-3 hours (under general anesthesia)

Post-ablation anticoagulation (at least 2-3 months)

Blanking period (first 3 months — early recurrences are not prognostic)

Rhythm follow-up with 3-month Holter or event recorder

Long-term anticoagulation decision (per CHA2DS2-VASc, independent of ablation success)
