What is Advanced Management of Peripheral Artery Disease?

Advanced peripheral artery disease (PAD) refers to severe atherosclerotic narrowing or occlusion of the lower limb arteries resulting in critical limb ischemia (CLI), with rest pain, ischemic ulcers and gangrene.

The Rutherford classification grades disease severity: category 4 is rest pain, category 5 is minor tissue loss, and category 6 is major tissue loss (gangrene). An ankle-brachial index (ABI) <0.4 suggests critical ischemia.

The primary goal is revascularization. Endovascular interventions (angioplasty, stenting, atherectomy) are considered first; surgical bypass is planned when anatomy is unsuitable.

A multidisciplinary vascular team, wound care, infection control and aggressive cardiovascular risk management are key to reducing amputation rates.

Symptoms

Leg pain at rest (especially when recumbent, relieved by dangling)
Non-healing foot and leg ulcers
Gangrene of toes or foot
Cold, pale leg
Absent peripheral pulses
Claudication (leg pain with walking)

Risk Factors

Smoking (strongest risk factor)
Diabetes (greatly increases amputation risk)
Hypertension and hyperlipidemia
Chronic kidney disease
Advanced age and male sex
Coexisting coronary and cerebrovascular disease

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Onset of leg rest pain (urgent evaluation)
  • Development of a non-healing wound on the foot or leg
  • Sudden cold leg with pulse loss (acute ischemia — emergency)
