What is Advanced LVAD/BiVAD Management in Heart Failure?

Left ventricular assist devices (LVAD) and biventricular assist devices (BiVAD) mechanically support the failing heart's pumping function in patients with advanced systolic heart failure. LVADs are used as a bridge to transplantation, a bridge to recovery or as destination therapy.

Modern continuous-flow LVADs (e.g., HeartMate 3) offer improved survival with lower rates of pump thrombosis and stroke. BiVAD systems are used when both right and left ventricular support is needed.

Advanced management requires careful anticoagulation titration, driveline care, infection control, surveillance for pump thrombosis, arrhythmia management and multidisciplinary heart failure team follow-up.

Risk Factors

INTERMACS profile 2–4
Transplant candidacy
Severe left ventricular dysfunction
Low cardiac index
Elevated pulmonary pressure
Renal and hepatic dysfunction
Poor medication adherence

  • Driveline site discharge or fever
  • Bleeding (hemorrhage, GI)
  • New neurologic findings
  • Palpitations and syncope
  • Hemoglobinuria or dark urine
