What is Advanced Diagnosis and Treatment of Cardiac Sarcoidosis?

Cardiac sarcoidosis is a systemic disease characterized by myocardial non-caseating granulomas that can cause conduction disease, arrhythmias, heart failure and sudden cardiac death. Up to 25% of patients with sarcoidosis have cardiac involvement.

Diagnosis requires a combination of detailed ECG, echocardiography, Holter monitoring, cardiac MRI (with LGE) and FDG PET/CT. Extracardiac sarcoidosis supports the diagnosis; in some patients endomyocardial biopsy is required.

Treatment combines immunosuppression (corticosteroids, methotrexate, azathioprine, infliximab) with arrhythmia management and ICD evaluation. Transplantation may be considered in advanced heart failure.
