What is Abdominal Aortic Aneurysm?

An abdominal aortic aneurysm (AAA) is defined as a dilation of the abdominal aorta to 1.5 times normal diameter or above 3 cm. It most often involves the infrarenal segment and is more common in men, smokers and older adults.

AAAs are frequently asymptomatic and discovered incidentally on imaging. Rupture risk increases with size; elective open surgery or endovascular repair (EVAR) is recommended above 5.5 cm.

Rupture presents with sudden severe abdominal or back pain, hypotension and a pulsatile mass, and carries very high mortality. Screening programs (ultrasound screening in men aged 65–75 with a smoking history) are an important preventive strategy.

Asymptomatic in most cases
Pulsatile abdominal mass
Back or abdominal pain
Rupture: sudden severe pain and shock
Hematuria from ureteral erosion
Signs of distal embolization
Early satiety due to compression

Risk Factors

Smoking
Hypertension
Dyslipidemia
Atherosclerosis
Advanced age
Male sex
Family history of AAA

  • Pulsatile back or abdominal pain
  • Rapid growth (>0.5 cm in 6 months) in a known aneurysm
  • Severe sudden abdominal pain with dizziness
  • Signs of limb ischemia
  • High-risk age group eligible for screening
