What is Roseola Infantum (Sixth Disease)?

Roseola infantum, also known as the sixth disease, is an exanthem disease caused by human herpesvirus 6 (HHV-6) and HHV-7. It affects infants between 6-24 months.

The clinical course is quite typical: during 3-5 days of high fever (39-40°C), the child usually appears restless only due to fever without general deterioration. Immediately after the fever drops, a pink maculopapular rash starts on the trunk and spreads to extremities.

The rash fades quickly without leaving any mark. Febrile convulsion risk may increase during high fever. Treatment is symptomatic and prognosis is excellent.

Symptoms

High fever lasting 3-5 days
Pink rash starting after fever drops
Maculopapular rash spreading from the trunk
Mild restlessness
Mild rhinitis
Nagayama spots (on palate)

Risk Factors

6-24 month age group
Daycare and crowded contact
Seasonal peak (spring-autumn)
Age of primary immune response
Immunodeficiency (more severe course)
Contact with non-immune sibling

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When high fever lasts longer than 3 days
  • When convulsion develops during fever
  • Infants who cannot feed or are excessively sleepy
  • When respiratory distress occurs with rash
  • After contact in immunodeficient children
