Neonatal Phototherapy

What is Neonatal Phototherapy?

Phototherapy is the first-line treatment method in neonatal jaundice. Blue light at 420-470 nm wavelength converts indirect bilirubin in the skin into water-soluble form via photoisomerization and accelerates excretion via bile and urine.

Decision to start treatment is made with nomograms (Bhutani curve). Bilirubin level, postnatal hour, and risk factors (prematurity, hemolysis, asphyxia) are considered.

LED, fluorescent, or fiber optic light sources are used. Double-surface phototherapy (from above and below) is more effective. Effectiveness depends on duration, distance, and skin area.

Symptoms

Jaundice (in skin and sclera)
Bilirubin level above the nomogram threshold
Hemolytic disease (ABO/Rh incompatibility)
Rapid bilirubin rise in preterm infant
Direct Coombs positivity
Glucose-6-phosphate dehydrogenase deficiency

Risk Factors

Prematurity
ABO or Rh hemolytic disease
G6PD deficiency
Asphyxia and sepsis
Breastfeeding jaundice
Asian ethnicity

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When the newborn has severe jaundice
  • When bilirubin rises above the threshold
  • Family history of jaundice and hemolytic disease
  • Kernicterus warning signs (lethargy, poor feeding, high-pitched cry)
  • Worsening of jaundice after discharge
