Intussusception (Bowel Telescoping)

What is Intussusception (Bowel Telescoping)?

Intussusception is the most common acute surgical emergency in children aged 3-36 months, most often in the ileocolic region. Its incidence is around 1-4/1000 live births. 90% of cases are idiopathic; in a small portion there is a 'lead point' such as Meckel's diverticulum, polyp, lymphoma.

Mesenteric lymph node hypertrophy associated with viral infections such as adenovirus and rotavirus may be a trigger. It is more common in healthy, well-nourished male children.

The classic triad (intermittent crying, vomiting, 'currant jelly' stool) is found in only 15% of cases. Ultrasound is diagnostic with 'target sign'. Hydrostatic/pneumatic reduction or surgery is used in treatment.

Symptoms

Sudden onset severe colicky abdominal pain
Intermittent crying episodes
Bilious vomiting
'Currant jelly' stool (late finding)
Sausage-shaped mass in right lower quadrant
Lethargy (sole symptom in some cases)

Risk Factors

Male gender (2:1)
3-36 months age
Viral infection (adeno, rota)
Rotavirus vaccine (slight increase)
Meckel's diverticulum
Lymphoma and polyp
