Infantile Colic

What is Infantile Colic?

Infantile colic is a paroxysmal crying picture affecting 10-30% of healthy infants, typically starting from the 2nd week of life and spontaneously resolving around 3-4 months. Wessel's 'rule of three': crying >3 hours per day, >3 days per week, lasting at least 3 weeks.

Its etiology is not fully understood. Possible causes include: gastrointestinal immaturity, gut microbiota, cow's milk protein allergy, lactose intolerance, gas, inadequate breastfeeding technique, parental stress, and temperament.

Alternative causes (UTI, OM, hernia, intussusception, fracture) must be excluded before diagnosis. The basis of treatment is family reassurance and supportive measures. Most cases do not require medication.

Symptoms

Sudden onset of intense crying
Drawing legs to the abdomen
Facial flushing and tension
Passing gas
Worsening in evening hours
Inability to soothe the infant

Risk Factors

First-time parenthood
Maternal anxiety and depression
Cigarette smoke exposure
Cow's milk protein allergy
Premature infant
Abnormal gut microbiota

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • When the crying pattern is atypical
  • If feeding problem or weight loss is present
  • If accompanied by fever, vomiting, or bloody stool
  • When parental burnout and mental health are deteriorating
  • In suspicion of hernia or abdominal distension
