What is Childhood Hypothyroidism?

Hypothyroidism is the inadequate production of thyroid hormones (T3, T4). It may be congenital (1/3000-4000 live births, detected on newborn screening) or acquired (autoimmune Hashimoto thyroiditis, iodine deficiency, surgery, radiation).

Untreated congenital hypothyroidism causes intellectual disability and growth failure. It is detected by newborn heel-prick screening, and levothyroxine is started early.

Acquired hypothyroidism most often results from Hashimoto thyroiditis, presenting with fatigue, weight gain, slowing growth, pubertal delay, and goiter. Lifelong levothyroxine replacement is the treatment.

Slowing growth and short stature
Fatigue and decline in school performance
Weight gain
Cold intolerance and dry skin
Constipation
Goiter and delayed puberty

Risk Factors

Family history of autoimmune thyroid disease
Down syndrome and Turner syndrome
Type 1 diabetes and celiac disease
Iodine deficiency
Radiation therapy
Amiodarone and lithium use

When to See a Doctor?

  • Marked slowing of growth velocity
  • Fatigue, constipation, and skin changes
  • Goiter on examination
  • Sudden drop in school performance
  • Pubertal delay
