What is Airway Foreign Body Aspiration in Children?

Airway foreign body aspiration (FBA) is a common paediatric emergency, peaking between 6 months and 4 years of age, with peanuts, sunflower seeds, raw vegetables, small toys and balloons as the most frequent culprits.

Aspiration may cause partial or complete airway obstruction. Complete obstruction at the glottis or trachea is immediately life-threatening; partial obstruction allows the child to breathe but causes wheeze, cough and stridor.

Most foreign bodies lodge in the right main bronchus due to its more vertical course. Long-standing aspiration leads to recurrent pneumonia, atelectasis, bronchiectasis and lung abscess.

Diagnosis is suspected from the history of choking episode, asymmetric breath sounds and unilateral wheeze. Inspiratory and expiratory chest X-rays may show air-trapping; CT and rigid bronchoscopy confirm and treat.

Symptoms

Sudden coughing, choking, gagging episode while eating or playing
Stridor (upper airway), wheeze (lower airway), unilateral chest sounds
Cyanosis, retractions, accessory muscle use, severe respiratory distress
Recurrent or non-resolving pneumonia in same lobe (chronic FBA)
Hoarseness, voice change (laryngeal foreign body)
Drooling, dysphagia (oesophageal foreign body, secondary tracheal compression)
Cardiac arrest if complete obstruction

Risk Factors

Age 6 months to 4 years (lack of molars, immature swallowing coordination)
Eating while running, laughing, talking
Access to small objects: nuts, seeds, hard candies, coins, button batteries, small toys
Older sibling sharing inappropriate foods
Lack of supervision during meals
Developmental delay or neurological disorder with poor swallowing
Ingestion of high-risk objects: latex balloons, magnets, button batteries

When to See a Doctor?

If you experience any of the following symptoms, seek medical attention promptly:

  • Sudden choking with cyanosis or inability to cough — call 112 immediately
  • Persistent cough or wheeze after choking episode requires urgent ENT/paediatric assessment
  • Recurrent pneumonia in same lobe — bronchoscopy indicated
  • Witnessed aspiration even without symptoms requires evaluation within 24 hours
  • Button battery ingestion is medical emergency — caustic injury within 2 hours

Treatment Methods

Conscious choking child <1 year: 5 back blows alternating with 5 chest thrusts

Conscious choking child >1 year: 5 abdominal thrusts (Heimlich manoeuvre)

Unconscious child: CPR with chest compressions, look in mouth before each ventilation

Rigid bronchoscopy under general anaesthesia is gold standard for retrieval

Flexible bronchoscopy may be used for distal airway, but rigid preferred for safety

Post-procedure observation for airway oedema, antibiotics for pneumonia, bronchodilators

Prevention: avoid nuts, seeds, popcorn, hard candy under age 4; supervised meals; small object education
